Before your first visit to Spine &
Sports Physical Therapy for therapy, you
may have some questions.
How do I get to your office?

What should I wear to
therapy?
Please
wear clothing which will allow
accessibility to the area of the body to
be treated, as well as comfortable and
supportive shoes.
How can I get started?
A referral
is required from your physician with a
recent date and diagnosis. Insurance
information is required to ensure proper
authorization and coverage verification.
We make every attempt to schedule your
initial appointment within 24-48 hours
of your request. Proper insurance
authorization is necessary to do so.
What should I bring to my
first visit?
Arrive to
your first appointment with your
prescription, insurance card, any
special test results and/or reports
pertaining to your injury, and
appropriate clothing which will allow
access to the body part(s) being
treated.
How long are visits?
Each visit
is approximately one hour in duration.
Your initial visit will consist of a
comprehensive evaluation by a licensed
physical therapist, who will then
individualize a treatment plan based on
your diagnosis.
What type of insurance do you
accept?
We accept most forms of private
insurance, Worker's Compensation Insurance, Medicare, and selected Health
Maintenance Organizations. Please see our
complete list of insurance carriers.
Often, even if we are not "in network"
with your private insurance carrier, we
can deliver treatment in a very
cost-effective manner. If your private
insurance is not listed, please call our
clinic and ask for the Clinic Business
Manager for more information. We are
also members of Access Medical Card PPO.
How will my physician follow my
progress?
Prior to
your physician appointments, we will
write a progress note which you will
hand carry to your physician. We will
also fax a copy to the physician's
office.

What if I need to cancel or
reschedule my appointment?
Spine &
Sports Physical Therapy is committed to
exceptional customer service in order to
enhance your recover from injury and
illness. In order to accomplish this
quality of care, it is extremely
important that you attend every one of
your scheduled appointments. Please
review our
cancellation policy.
Can I be seen without a prescription?
Yes, one
can be evaluated and seen without a
prescription, on a cash-pay basis.
